All San Juan IPA Members/Office Managers: Submitting Incomplete Credentialing Applications
Starting in March, 1 2020, San Juan IPA will now be required to send all applicants notification of missing and incomplete items via certified US mail in order to comply with the New Mexico Administrative Code. Returning an incomplete application will now cost applicants an additional $25.00, and may create potential delays in presenting your files to our committee. It is imperative to follow the checklist provided on the applications and fully complete every line item of the application to avoid this additional fee. In addition, please be sure to allow plenty of time prior to your due date for review and processing.
