Update

The City of Farmington has contracted with Reliance Medical Group to provide urgent care services for its members. Reliance is located at 3451 N. Butler Ave. Farmington. The Phone Number is 505-566-1915. The Reliance Urgent Care Co-payment is $20.00 and insurance pays 90% of the bill after the deductible has been met.

New Mental Health Providers

The City of Farmington has contracted with Mines and Associates to provide Mental Health Services for it's employee's. If you are in need of a referral for Mental Health Services please call the prior authorization request number at 1-800-873-7138.
