"The San Juan IPA’s Health Promotion Program enables our customers – employers, healthcare providers and insurers – to improve quality of life, create economic opportunity and increase competitive advantage with better, more accessible and more cost-effective healthcare for everyone in the communities they serve." The San Juan IPA’s, Health Promotion Program is a comprehensive disease prevention and chronic disease management solution. It enables employers, health care providers and insurers to realize substantial and lasting improvements in the health of their employees and the communities they serve. The San Juan IPA’s Health Promotion System provides:  A tested and proven methodology which has produced substantial improvements in workplace and community health over the last thirty years.  "Smart Software" to guide the health management program, evidence-based clinical content to inform it and the power and connectivity required to implement and sustain it cost-effective with large groups of people in a reasonable period of time. The San Juan IPA’s Health Promotion Program uses the Franklin Health Model as its approach. Developed and perfected over the last thirty years, this approach has produced substantial and documented improvements in health behaviors, risk factor control, perceived quality of life, health resource utilization (such as preventable hospitalizations), reduced healthcare costs, improved life expectancy and lowered death rates. The San Juan IPA’s Health Promotion Program starts with an initial 45 minute encounter between the client (and employee, a patient or a community resident) and the personal health counselor, (a nurse, medical assistant, health educator, or nutritionist). The San Juan IPA’s Health Promotion Program software guides the counselor through the encounter process, integrating evidence-based clinical guidelines with each client’s unique demographics, health history, past and current risk factor values and current risk factor treatment status. It also tracks each encounter for individual follow-up and evaluation. Additionally, the System software powers the assessment, tracking and management of the health of individuals, worksites, and communities across many locations.

Philosophy

San Juan IPA supports a philosophy toward the treatment of the patient with an illness (usually chronic in nature) that seeks to prevent recurrence of symptoms, maintain high quality of life, and prevent future need for medical resources by using a comprehensive approach to health care. Pharmaceutical care, continuous quality improvement, practice guidelines, and care management all play key roles in this effort, which should result in decreased health care costs as well.

Wellness Programs

Exercise Wellness We have a program put together by physical therapy professionals, it is a powerful exercise management tool designed to create individual exercise plans for specific fitness goals. Weather it be aerobic endurance or to power up a golf swing. Exercise routines can be easily created in minuets with printed workout cards which track exercise performance and show how each exercise is done. Exercise routine schedule and equipment choices are extremely flexible. Comprehensive personalized progress reports are generated with each follow-up and exercise plans can easily be changed as goals change. Diet We also have a program put together by dietary professionals for weight management. This program performs four functions, which are monitored by the Clinical Dietician that we employ: 1. First dietary analysis is preformed to help the employee understand their eating habits and what is needed to improve. The analysis report compares current calorie, protein, carbohydrate, and fat intake to recommend intake levels. Analysis of nutrient intake identifies current vitamin and mineral deficiencies. 2. Using foods from our database custom meal plans can be built from scratch. Food alternatives within meal plans can easily be substituted from an exchange list if tastes change. 3. Shopping lists are generated automatically with meal plans and are adjustable to the number of people who will be dining with you. 4. Employee’s progress is evaluated every 2-3 weeks by confirming the rate of weight change and comparing it with the employee’s goals. Smoking Cessation Based on the American Lung Associations Stop Smoking Program our classes incorporate proven methods to success. We will offer a comprehensive program, two nights a week, over three and a half weeks that teaches the basics for successfully kicking tobacco addiction. In addition, we will set up support groups that are specific to your organization. Stress Management This program teaches each individual how to manage and relieve unnecessary stress. The program is based on proven methods including: Atmospherics, Biofeedback, meditation, and exercise techniques as wide ranging as aromatherapy to kung-fu. Individuals are taught how to identify what works for them from a myriad of options and how to channel unnecessary stress into efficiency.
